Project Title: Darlington Communities (not approved project)

Project Description:
The aim of the project is to improve the health of local residents by providing accessible community health activities underpinned by initiatives to tackle the root causes of poor health . The theme of the project has been identified across the different areas involved in the Darlington Communities project as one problem where a centrally co-ordinated effort would be required to make a sustained impact. As a result, Darlington Primary Care Trust will work with each community partnership to identify joint priorities and specific community needs. This will be developed into a community health plan which will deliver targeted health projects across the deprived communities, through the established community partnerships.

The objectives of the project are: to improve take up of healthy lifestyles through the provision of advice and a co-ordinated programme of activities, to support the health needs of vulnerable people in the community and to improve access for parents to advice and assistance to reduce levels of problems affecting children.
